St Francis Subiaco has joined thousands of healthcare providers across Australia in registering with MyMedicare.
MyMedicare is a voluntary patient registration model. It aims to formalise the relationship between our patients, and our general practice and general practitioners.
Seeing your GP regularly and formalising the relationship you have with your GP and general practice through MyMedicare can lead to better health outcomes.
By registering as a patient at St Francis Subiaco, and selecting your preferred GP, additional funding will be available from the government to assist your primary care team to deliver the care you need.
If you have a valid Medicare card, or Department of Veterans’ Affair (DVA) Veteran card and have had two face-to-face consultations at St Francis Subiaco in the last 24 months, you are eligible for MyMedicare.
- A formalised relationship with St Francis Subiaco or your GP, which has shown to IMPROVE health outcomes.
- Longer Medicare Benefit Scheme (MBS) funded Telehealth consultations with your GP.
- Incentive for longer Telehealth consultations for children under 16 and valid Commonwealth Concession card holders.
- More regular visits from your GP and better care planning for people living in a residential aged care home, from August 2024.
- Connections to more appropriate care in general practice for people who visit hospital frequently.
To register as a MyMedicare patient, you can:
- Start the registration process in your Medicare Online Account or Express Plus Medicare Mobile App. Our practice staff will then accept the registration in the MyMedicare system.
- Alternatively, complete the online HotDoc Registration Form, click here. One of our Receptionist will then process the registration on your behalf.
